Corrective Action: Effective 06/09/2026, S1 stated she will transition the three youngest children into Room 3, to ensure compliance with this regulation.

Based on record review at 11:28 a.m., S1 failed to ensure C5, 5-years-old, prescription medication, Epinephrine 0.15 MG, did not have an expired date. The specialist observed the Epinephrine expiration date of 04/30/2026. This could not be corrected. Corrective Action: Effective 06/09/2026, S1 stated she will document expiration dates on calendar to inform parents 30 days prior, to ensure compliance with this regulation.

The ratio was corrected when S11 went into the classroom at 9:42 a.m. Corrective Action: Effective 10/14/2025, S1 stated she will review ratio with all staff at the next staff meeting on 10/30/2025, to ensure compliance with this regulation.
