One child, aged 2 years 4 months, in Room 8, was not actively supervised by the staff member when they were tending to children in the bathroom; therefore allowing the child the opportunity to open the classroom door to a stranger unnoticed.

During the onsite visit on 12/10/25 staff in Room 5 did not wash their hands after diapering.

During the onsite visit on 12/10/25 the diaper changing surface in Room 5 was not disinfected after each use when the facility is using a commercial product that cleans & deodorizes but does not disinfect.

During the onsite visit on 12/10/25 the food prep area in Room 5 is not located away from the diaper changing area when bottles used for consumption are stored on the diaper changing table, filled with water, and given to children to drink.

During the onsite visit on 12/10/25 bottles used for children are not maintained in a sanitary manner when they are only rinsed out with water between use instead of being washed and sanitized after each use.

Lesson plans were not current in two classrooms.

Two boxes of cereal were open in the kitchen and were not stored in a sealable plastic bag or container with a tight-fitting seal.

Out of ten children's files reviewed, two children do not have documentation of current immunizations on file.

Out of ten staff files reviewed, one staff did not have work references on file.

Facility allowed two individuals to work in the facility while background checks remain pending.

Child's hands were not washed after being diapered in Room 4.

Room 5 did not have toys in sufficient quantity to allow children a choice of three activities.

Room 5 did not have toys readily accessible to the children when the toys are stored on a higher shelf.

Two bottles with breast milk were not labeled with contents and time expressed.

Two bottles in Room 4 did not have the nipple of the bottles covered.

The minimum staff-child ratio was not maintained in the infant room when there were four infants with one staff.(Corrected when another staff arrived.)

One staff is working and does not have completed results of the background.

One of 17 child files reviewed do not have a completed emergency authorization form on file.

Upon licensing specialists arrival, 5 children, 2 1/2 to 3 years of age, were not supervised in Room 1 (Monkeys), when the staff was observed leaving the children in the classroom to gather three children off of the playground.

Diaper procedures were not followed in the Koala's unit when staff did not remove the paper towel from under the child's bottom (Step 6)and did not remove the gloves before placing the clean diaper on the child. (Step 7)

Inappropriate discipline was used when a staff member was heard saying, "No. You have been bad. You will not get supper tonight" in Room 1.

A common towel is used for wiping down the diapering surface between children.

Staff in five classrooms did not sit with the children at lunch to encourage socialization. Children were not encouraged to serve themselves in four classrooms.

One bottle was not refrigerated when stored on a changing table in room 6 then moved to a shelf in room 7. The nipple was not covered and was not labeled with the date it was prepared or the contents.

One unit does not have a staff who meets the minimum qualifications for the unit.
