The facility's transportation log did not include [date, time in & time out, and signatures for yesterday]. CCF Handbook, Section 2.5.2 (Section 2.1 Health and Safety, Page 3) General Requirements [SR] The driver completed the transportation log for yesterday, and the 2nd adult signature. The driver and 2nd adult confirmed that a visual sweep was conducted by both of them yesterday. Technical support was provided to both drivers and the Director. 2.5.2 Transportation Log A. A log must be maintained for all children being transported in a vehicle or on foot away from and/or to the premises of the child care facility. The log must be retained on file at the facility for a minimum of 12 months and available for review by the licensing authority. The log must include: 1. Each childs name, 2. The date and time of departure, 3. Time of arrival at the destination, 4. The signature of the driver (or in the case of travelling on foot, the signature of the child care personnel), and 5. The signature of a second child care personnel or person(s) authorized by the provider to verify the transportation log and that all children have arrived safely and left the vehicle (if applicable).

Child care personnel failed to ensure arrival and departure times were complete and accurate at the time of arrival or departure. CCF Handbook, Section 7.5, B The three y/o classroom teacher did not have an attendance roster for today. She marked all 12 children as present during this inspection. The 1 y/o classroom only had five children marked as present; six children were present. The teacher marked the 6th child in as present during this inspection. Technical support was provided. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom.

A potentially harmful item, [bleach water solution and Windex], was not labeled as required. CCF Handbook, Section 3.2, B (Section 10, number 4) Physical Environment [SR] The (2 y/o) teacher labeled both bottles of cleaner during this inspection. Technical support provided. 3.2 Toxic/Hazardous Material/Firearms/Weapons B. All potentially harmful items, including cleaning supplies, flammable products, poisonous, toxic, and hazardous materials, must be labeled and used according to manufacturers recommendation. These items, as well as knives, sharp tools, and other potentially dangerous hazards, must be stored in a locked area or must be inaccessible and out of a childs reach at all times.

CCF Handbook, Section 7.5, B Two classrooms did not have an accurate attendance and one classroom had not completed her classroom attendance yet this morning. The (three) teachers completed an 'accurate' attendance record during this inspection. Technical support provided. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program. However, child care facility personnel are responsible for ensuring that attendance records are complete and accurate.

Child care personnel failed to document an accident or incident on the day that it occurred. CCF Handbook, Section 6.4, A Health Requirements [SR] An Accident Report was completed during this investigation, including a parent signature. Technical support provided. 6.4 Accident/Incident Notification A. All accidents and incidents or unusual occurrences that are threatening to the health, safety, or welfare of a child that occur while the child is in care must be documented on the same day they occur. B. This documentation must be shared with the custodial parent or legal guardian on the date of occurrence. C. Documentation must include the name of the affected party, date and time of the occurrence, description of the occurrence, actions taken and by whom, and appropriate signatures of program child care personnel and the custodial parent or legal guardian. Program child care personnel signatures may include the director/child care personnel that witnessed the incident, who were involved in the incident, and/or responded to the childs needs. D. The documentation must be maintained for 12 months. If the parent or legal guardian does not pick up the child on the date of occurrence of the accident or incident, the individual authorized to pick up the child must sign and be provided a copy of the accident/incident form.

The facility did not have a fully-completed enrollment form (CF-FSP Form 5219) or equivalent form for the child(ren) in care. CCF Handbook, Section 7.3 Record Keeping [SR] The parent completed an Enrollment form for the child during this investigation. Technical support provided. 7.3 Enrollment Information The facility operator shall obtain enrollment information from the childs custodial parent or legal guardian prior to accepting a child in care. This information shall be documented on CF-FSP Form 5219, Child Care Application for Enrollment, which is incorporated by reference in 65C-22.001(7)(f), F.A.C., or an equivalent form that contains all the information required by the Department on CF-FSP Form 5219. CF-FSP Form 5219 may be obtained from the licensing authority or on the Departments website at www.myflfamilies.com/childcare. A. Enrollment information shall be kept on file, current and available for review by the licensing authority. B. The enrollment information shall include, in writing, permission for the facility to release the child to any person(s) authorized or in the manner authorized by the custodial parent or legal guardians. The name, address, and phone number of authorized persons must be in the enrollment information.
