Based on observation/record review at 10:15 a.m., S1 failed to ensure the center's daily attendance record for children accurately reflected the children on the child care premises at any given time. There were 23 children present and 26 children were signed in on the log. This was not corrected prior to the Specialist departure. Corrective Action: Effective 7/29/2025, S1 stated she will check her daily attendance logs to make sure they are accurate to ensure compliance with this regulation.

Based on record review at 11:00 a.m., S1 failed to have documentation that all staff members annually completed the online child abuse and neglect Mandated Reporter Training provided by DCFS. S1, S2, S3, and S4 failed to have documentation of completing the online Mandated Reporter Training annually. S1's last training expired on 7/19/2025. S2's last training expired on 9/20/2024. S3 and S4's last training expired on 7/23/2025. Corrective Action: Effective 7/29/2025, S1 stated all staff will take the training on 8/1/2025 to ensure compliance with this regulation.

C.: Based on record review at 11:00 a.m., S1 failed to have documentation that the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. The last visual check documented was done on 11/1/2024. Corrective Action: Effective 7/29/2025, S1 stated she will ensure the visual checks are completed at the end of the day to ensure compliance with this regulation.

Based on observations/record review at 12:00 p.m., S1 failed to ensure substitutions or additions were posted, written or electronically, on or near the menu. The menu posted stated the children would be served chicken sticks, mac and cheese, mixed vegetables, rolls, applesauce and milk, but the children were served chicken nuggets, mac and cheese, corn, peaches, and milk. This was not corrected prior to the Specialist departure. Corrective Action: Effective 7/29/2025, S1 stated she will make substitutions are posted to ensure compliance with this regulation.
