      Based on record review, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. LPA Uribe observed that out of the 10 children's files reviewed, 9 of them had at least one incomplete licensing form. LPA Uribe discussed in depth which child was missing what information on which forms with the licensee. POC Due Date: 01/07/2022

The rope ladder to on the Large Lifetime play structure & all of the geometric shaped climbing play structure are not properly secured to the ground. POC Due Date: 01/07/2022
