      Based on record review, the last time a fire drill was completed was 12/16/2022. Based on this information, the licensee did not comply with the section cited above in which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 12/12/2023

    • Responsibility for Providing Care and Supervision for Infants (B) Staff Shall Physically Check on Sleeping Infant(s) Every 15 Minutes and Document the Following: This Requirement…CCR 101429(a)(2)(B)

      Based on record review and staff interviews, staff do not conduct or document 15-minute sleep checks for infants between the ages of 12 to 24 months. Staff stated they were unaware that sleep checks should be completed and documented after an infant turns12 months of age.       Based on LPAs observation and record review LPAs observed Staff #3 caring for four infants alone. LPAs reviewed Staff #3's file and saw the file did not contain any documented units completed in Early Childhood Education or Child Development.       Based on LPAs observation. During file review LPAs observed files of six infant children who's Infant Needs and Servies plan have not been updaetd quarterly as required by Title 22 reglations.
