Based on observation, interview, record review, the licensee did not comply with the section cited above facility is using the room #1,#2,#3, #4 (classrooms located by the office room and addition classroom by the swimming pool for the ealry interventation program. Per Assistant Director, these classrooms is licensed from Dept. Social Services which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 03/30/2023
