Supervision of children in the [VPK class] group was inadequate in that [a child was left in the bathroom and a check off of attendance was not conducted,]. CCF Handbook, Section 2.4 General Requirements [SR] During todays inspection a child in the VPK class group was left in the bathroom while the class went to the playground. At the time of inspection, the teacher went back to the classroom and brought the child outside with his VPK class. Technical assistance was given on checking off attendance before and after the playground. Technical assistance was given. CCF Handbook, Section 2.4 2.4.1 General Supervision Requirements A. Childcare personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. Childcare personnel must be assigned to provide direct supervision to a specific group of children and be with that group of children at all times. Children must never be left without childcare personnel supervision inside or outside the facility, in a vehicle, or at a field trip location. Due Date Completed at time of inspection

A ratio of [1 ] child care personnel for [4 infants] children is required. A ratio of [1] child care personnel for [1 infant and 6 two year olds] children was observed. s.402.305(4), F.S. General Requirements [SR] • At today's inspection the two-year-old room was observed with an insufficient ratio: During naptime there were 6 two-year-old children and 1 infant present. • The director called the infant teacher who was on lunch break to return to the center. The ratio was put back into compliance with 6 two-year-old children, 1 staff.

The Student Health Examination or equivalent health statement was not acceptable in that: [ It was not current (over two years old)]. CCF Handbook, Section 7.2 (Section 19.2, number 1) Record Keeping [SR] FSC reviewed all 49 preschool children's records. The two children listed on the Supplemental Sheet had expired (over two years) Student Health Exams/Physical forms. Technical support was provided. 7.2 Student Health Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Student Health Examination form DH 3040, which is incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, the parent or legal guardian, or a signed statement by authorized professionals that indicate the results of the components of the Student Health Examination form are included in the health examination. A. The Student Health Examination shall be completed by a person given statutory authority to perform health examinations. B. The Student Health Examination or the signed statement is valid for two years from the date the physical was performed. An up-to-date version must be on file for as long as the child is enrolled at the facility. C. If the custodial parents or legal guardians fail to provide the documentation required above within 30 days of enrollment, the facility shall not allow the child to remain in the program.

The facility made use of double or multi-deck cribs, cots or beds for children younger than one year old. CCF Handbook, Section 3.6.1, I Physical Environment [SR] The FSC observed a child sleeping in bouncing chair. This was resolved when the staff member put the child in a crib. Technical assistance: 3.6 Napping/Sleeping Requirements For the purposes of these requirements, sleeping refers to the normal night-time sleep cycle, while napping refers to a brief period of rest during daylight or early evening hours. 3.6.1 Bedding and Linens I.
