      Based on observation at 11:45 a.m., S1 failed to ensure all outdoor hazards were inaccessible to children. The Specialist observed several wooden pieces from the outdoor fence were missing and broken causing sharp pieces/nails to be exposed and accessible to children. Corrective Action: Effective 4/30/2026, S1 contacted a contractor prior to Specialist departure to schedule repair service, to ensure compliance with this regulation.

      Based on observation at 11:30 am, S3 failed to ensure pacifiers were not attached to a child. The Specialist observed C1, eight-month-old, with a pacifier attached to her clothing. Corrective Action: Effective 4/30/2026, S1 stated she contacted C1's parent to ask that she remove lanyard from pacifier, prior to child entering the center, to ensure compliance with this regulation.

Based on record review/interview at 9:30 a.m., S4 failed to document the entire center and play yard is checked after the last child departs to ensure that no child is left unattended at the center. The last visual check of the center was conducted on 4/29/2025. Corrective Action: Effective 5/06/2025, S4 will place the end-of-day check at the front door as a reminder to complete the check daily, to ensure compliance with this regulation.
