Corrective Action: Effective 10/6/2025, S2 stated she will have all visitors that com on premises to sign in and out to ensure compliance with this regulation.

1915.A. Based on record review at 11:15 a.m., S1 failed to have documentation of observations, when something is observed, noted on children upon arrival to the center. Results including an explanation from parent and/or child were not documented. S1 stated on 8/27/2025, C1, age 1-year-old, was observed having small bumps on his hands and face, but there was and no documentation of the observation. Corrective Action: Effective 10/6/2025, S2 stated she will review the observation logs by 10:00 a.m. daily for proper documentation to ensure compliance with this regulation.
