1727.A. Based on record review at 10:34 a.m., S1 failed to have documentation that S5 completed the required annual Online Child Abuse and Neglect Mandated Reporter Training provided by DCFS. S5’s certificate expired on 02/10/2026. Corrective Action: Effective 03/12/2026, S1 stated she will set a reminder 30 days prior to the certification expiration and ensure the trainings are from approved providers to ensure compliance with this regulation.

1807.B. Based on record review at 10:21 a.m., S1 failed to have documentation of said eligible determination available for S11 (DOH 01/20/2026) on the centers CCCBC roster at all times. This was not corrected prior to Specialist departure. Corrective Action: Effective 03/12/2026, S1 stated she will remind set a daily reminder to check CCCBC to ensure staff are on the roster to ensure compliance with this regulation.

1507.E. Based on record review/observation at 9:45 a.m., S2 failed to maintain documentation of a daily attendance record for Visitor's to include first and last name, date of visit, arrival and departure times, and purpose of the visit. The Specialist observed O2 walking in and out the building, twice, to gain access to the play yard with no documentation of signing in. Corrective Action: Effective 10/6/2025, S2 stated she will have all visitors that com on premises to sign in and out to ensure compliance with this regulation.

1915.A. Based on record review at 11:15 a.m., S1 failed to have documentation of observations, when something is observed, noted on children upon arrival to the center. Results including an explanation from parent and/or child were not documented. S1 stated on 8/27/2025, C1, age 1-year-old, was observed having small bumps on his hands and face, but there was and no documentation of the observation. Corrective Action: Effective 10/6/2025, S2 stated she will review the observation logs by 10:00 a.m. daily for proper documentation to ensure compliance with this regulation.

705.A. Based on observation at 9:50 a.m., Licensing Division staff was not admitted into a center immediately, without delay and was not given free access to all areas of a center, including the grounds. S3 and S4 refused to open the door for the Specialist while observing the Specialist through the front door. S7 stated to the Specialist the staff is probably calling S2 to notify her you are here. This was corrected prior to Specialists departure. Corrective Action: Effective 09/18/2025, S1 stated she will check the CCCBC roster daily to ensure all staff have a current CCCBC to ensure compliance with the regulation.
