Based on record review and interviews on 6/16/2025, at 12:50 p.m., S1 failed to notify the Department within 24 hours of the following critical incident: On 6/11/2025, at around 8:00 a.m., O1 reported to S4 that C1, 4-years-old, disclosed she was inappropriately touched by C2, 4-years-old, while present at the center. S4 subsequently informed S1 of the allegation. S1 then informed S27 and requested that she follow up with O1. S27 contacted O1 on 6/11/2025, at 10:00 a.m. to discuss the allegation.
