Based on observation and interview with staff, the licensee did not comply with the section cited above in LPA observed floors to be dirty stained carpeting and crumbs on the floor in the infant areas. LPA interviewed a staff who admitted to not using shoe covers or changing out her shoes where children were sleeping on a mat on the floor which poses a potential health, safety or personal rights risk to persons in care. Based on record review, the licensee did not comply with the section cited above LPA observed sleep plans to be incompleted without authorized representative signatures nor staff signature.
