Based on observation, the licensee did not comply with the section cited above. During the outdoor inspection, LPA observed a children play structure consist of two towels on each end connected with a crawl tunnel between the tunnels. LPA observed duct tapes were being used to cover the outside and inside of the tunnel. Per Site Supervise, the duct tapes were used to cover multiple cracks (damage) on the crawl tunnel. This poses a potential health, safety, or personal rights risk to children in care. POC Due Date: 08/16/2024
