      Based on record review, the licensee did not comply with the section cited above in 2 out of 2 staff did not have current mandated reporter training which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 08/17/2023

      Based on observation, the licensee did not comply with the section cited above in two children were ibserved going to the restroom by themselves and one child closed the bathroom door. Staff confrimed that children go to the bathroom alone; which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 10/19/2022
