This Statement of Deficiencies was generated as a result of the on-site State licensure inspection conducted at your facility on 04.02.2026. Please respond to each deficiency and attach documents as requested for the deficiency it pertains to. Sign and submit your Plan of Correction within 10 business days of receipt. Inspection census, the facility is licensed for 201 children as a Center. The census at the time of survey was 49 children. 15 children's files and 18 staff files were reviewed. Based on observation and interview, the facility was found to have hazards as listed below: Surveyor observed that the area around the trees had dips where children's feet could get stuck or pose a tripping or falling hazard. Please fill in the area with gravel or woodchips so that it is level with the playground surface so that children don't accidentally fall or get their feet caught in the "dips" around the trees. Surveyor counted 9 trees where the areas have to be filled. Please upload pictures as evidence of correction. Answer the 4 POC questions. Based on interview and record review, the facility failed to maintain a daily sign-in sheet that included the times of arrival and departure for staff. Surveyor observed that 2 out of the 3 staff were not signed in in room #8. In addition, surveyor observed that in rooms #8 and #1, the staff had pre-filled their time in and out for breaks and lunches. To ensure times are consistently and accurately recorded, please provide instruction to staff and parents and monitor the daily sign-in sheets. There were no regulatory deficiencies identified at the time of the survey. The facility is licensed for 201 children as a Center. Reminder: Ensure that all adult scissors are stored or locked out of reach and inaccessible to children. It should be noted that adult scissors were observed in reach in one classroom and this hazard was corrected on site.
