      Based on observation and interview, the licensee did not comply with the section cited above in 1/1 napping infants were wearing a sleep sac during nap in crib, which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 12/11/2025

      Based on interview and observation the licensee did not comply with the section cited above in 1/1 play structures on the shared playground has broken plastic legs and holes in the plastic throughout the base of the structure that connects to the rubber mat, areas approx larger than 14inx14in once stepping onto the base of the structure also are covered with wood that is in disrepair and peeling plastic edging throughout the structure, which all pose a potential health, safety or personal rights risk to persons in care.
