On 5/12/2026, at 1:48 p.m., S1 was observed on video pulling C2, 2-years-old, aggressively by his left arm. This could not be corrected. C.: Based on record review on 6/17/2025, at 10:00 a.m., S1 failed to notify the Department within 24 hours of the following incident: On 6/5/2025, at 6:12 p.m., O5 sent a message through the parent communication app, asking what happened to C2, 1-year-old, because he was limping. S5 responded on 6/6/2025 at 7:45 a.m., nothing happened and he had a great day. S1 reached out to O5 on 6/6/2025, to set up a parent meeting to discuss concerns.
