Child/ren were placed in a crib, playpen, play yard or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. CCF Handbook, Section 3.6.4, A Physical Environment [SR] Licensing specialist observed crib mattress covers were loose fitting and not appropriate for infant children. Provider ordered the appropriate bedding at time of inspection. 3.6.4 Safe Sleep A. All personnel that care for infants must follow safe sleep practices as recommended by the American Academy of Pediatrics (AAP) as referenced in Caring for Our Children Basics Health and Safety Foundations for Early Caremand Education, which is incorporated by reference in 65C- 22.001(7)(v), F.A.C. Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. The play equipment was not maintained in a safe or sanitary condition for the children to use in that [stationary metal equipment (red airplane & red dome climbing structures) had peeling paint]. CCF Handbook, Section 3.12, B and F Sanitation and Equipment [SR] Licensing specialist observed stationary metal equipment (red airplane & red dome climbing structures) had peeling paint. Provider removed at time of inspection. 3.12 Outdoor Equipment B. All playground equipment must be securely anchored, unless portable or stationary by design, in good repair, maintained in safe and sanitary condition, and placed to ensure safe use by the children. Maintenance must include inspections conducted every month of all supports above and below the ground and of all connectors and moving parts. Documentation of maintenance inspections must be maintained for 12 months. F. All equipment used in the outdoor play area must be constructed and maintained according to manufacturers recommendations and allow for water drainage. Bedding and/or linens were not stored in a sanitary manner. CCF Handbook, Section 3.6.1, G & H Physical Environment [SR] Specialist observed bedding in the bathroom. Staff placed bedding in a closet for safe and sanitary storage at time of inspection. 3.6.1 Bedding and Linens Each child in care must be provided safe and sanitary bedding to be used when napping or sleeping. Nap bedding is not required for school-age children; however, the program must provide an area for children that choose to rest. G. Bedding and linens, when not in use, must be stored in a sanitary manner which prevents the spread of germs or lice from other linens. All bedding and linens must be thoroughly cleaned and sanitized at least once a week, and before use by another child. H. Bedding and linens shall not be stored in the bathroom, unless stored in cabinets.

Attendance records did not include the time of each childs arrival and departure. CCF Handbook, Section 7.5 Record Keeping [SR] Specialist observed the attendance in the three's room did not have a time in for students. Staff completed at time of inspection. 7.5 Daily Attendance Daily attendance of children must be taken and recorded accurately by the child care personnel, documenting the time when each child enters and departs the program. A. Attendance devices used for the purposes of tracking attendance may be used, but personnel must ensure the accuracy of the documented attendance. Each classroom must have an attendance sheet/class roster for the group of children occupying that space. Attendance sheet/class roster must accompany the child care personnel and the group of children throughout the day should they leave the classroom. B. The custodial parent or guardian may document the time when his/her child enters and departs the child care facility or program. However, child care facility personnel are responsible for ensuring that attendance records are complete and accurate. C. REMOVED. D. Attendance records must be maintained for a minimum of 12 months. E. Attendance records for Voluntary Pre-Kindergarten or School Readiness may be used, if applicable.
