Based on record review/interviews on 4/16/2025, at 1:23 p.m., S1 failed to notify LDOE and DCFS within 24 hours of the following critical incident: On 4/9/2025, O1 came to the center making allegations of S6 abusing C1, age three-years-old, resulting in the police being notified on 4/10/2025. LDOE was notified on 4/14/2025. S1 notified DCFS during the licensing visit. Corrective Action: Effective 4/28/2025, S1 stated she will send all allegations and incidents the day of the report to the appropriate agencies within 24 hours to ensure compliance with this regulation.
