This Statement of Deficiencies was generated as a result of the on-site State licensure survey conducted at your facility on 02/09/2026. Please respond to each deficiency and attach documents as requested for the deficiency it pertains to. Sign and submit your Plan of Correction within 10 business days of receipt. The facility is licensed for 42 Children as a Center. The census at the time of the survey was 2 children and 1 staff. Files were not reviewed during this inspection visit but will be reviewed at the next inspection. If deficiencies are cited, an approved plan of correction must be 3. Director will make sure to keep a running log of each drill with signatures and dates. Based on observation and interview Children and staff will conduct each drill during inspection, fire drill and disaster according to the schedule listed on the drill drill logs were not provided and director logs. 4. Person responsible: Center Director stated they had not been conducted due to low enrollment. Fire drills must be completed and logged monthly, and disaster drills must be completed and logged quarterly. Please conduct and log both a fire drill and disaster drill for the month of February 2 Based on observation during inspection, it was observed that the carpet cleaning log was not provided. Please ensure that carpets are cleaned and logged in quarterly (every 3months). Please clean and log carpets for the month of February 2026 and upload a photo of the log to the Plan of Correction by 02/27/2026. Ensure that you answer all 4 questions in the Plan of Correction.
