      Based on the interview/record review, the Licensee did not meet 101216(g)(1) Personnel Requirements regulation which poses a potential Health, Safety & Personal Rights risk to the children in care. During the record review LPA was unable to review health screening or TB results for S1 and S4. Director stated health screenings and TB results are not available for review.
