Based on observation, the ceiling near the kitchen door was observed as drooping. Although it is not actively dropping, the ceiling piece was overlapped and there was a hole, exposing the inside of the ceiling. This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 05/31/2024

Based on LPAs observation, the door to the kitchen area was not locked, and door was open. LPA noted a box of baking soda under the kitchen sink. Additionally, there is a basket across the entrance/exit door which contains "PPE to go" packets. Both, the baking soda and PPE packets have a warning label to contact poison control if consumed/ingested and were accessible to children in care. This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 05/17/2024
