Based on record review at 12:45 a.m., S2 failed to ensure the center's daily attendance record for children accurately reflected the children on the child care premises at any given time as 81 children were present and 78 children were signed in. This was corrected. Corrective Action: Effective 5/27/2026, S1 stated she will have each staff check brightwheel on their phones, by 9:30 a.m., to ensure all children have been signed in and to ensure compliance with this regulation.

Based on observations at 12:45 p.m., S4 failed to ensure the required child to staff ratio for children was met. Specialist observed S4 supervising six children aged 6 weeks-to-1-year old. The required ratio for children of this age is 5:1. This was corrected when one child was moved to S7's classroom. Corrective Action: Effective 5/27/2026, S1 stated she will discuss with all staff that they must adhere to the posted child to staff ratio at all times to ensure compliance with this regulation.

5.: Based on record review at 1:30 p.m., S1 lacked documentation that 1 of 12 staff, S7 (first day working 7/28/2025), had completed LDE Key Training Module 3. This training should have been completed by 8/28/2025. This was not corrected. Corrective Action: Effective 5/27/2026 S1 stated she will have all new staff take a picture of completed training certificates as a backup and to ensure compliance with this regulation.

Based on observations, record review, and interview at 2:00 p.m., S1 failed to have documentation 1 of 12 staff on the premises and accessible to children have current certification in infant, child, and adult CPR and pediatric first aid through training approved by the Department and was being supervised until the training was complete. S12 failed to have the current certification. A class has been scheduled for early June 2026. 1907.E.2. Based on observations at 12:45 p.m., S4 failed to ensure cribs were free loose bedding (blankets) while a child was in the crib. The Specialist observed C1, 8-months-old and C2, 9-months-old, each in their cribs with blankets. This was corrected. Corrective Action: Effective 5/27/2026, S1 stated she will talk with parents and reiterate with teachers that blankets are not allowed while children are in the cribs to ensure compliance with this regulation.
