      Based on record review and interview at 12:00 p.m., S1 lacked documentation that S16, first day working 10/16/2025, completed the LDE Key Training Module 3 within 30 calendar days of their first day present at the center. This training should have been completed by 11/15/2025. This was not corrected prior to the Specialist's departure. Corrective Action: Effective 1/14/2026, S13 stated they have streamlined orientation training to include completing the modules before any new staff is allowed to work inside the classroom to ensure compliance with this regulation.

      C.: Based on record review and interview at 12:00 p.m., S1 failed to ensure 1 of 25 staff obtained a minimum of 12 clock hours of continuing education hours. S7 had 5 continuing education hours for the center's anniversary year from November 2024 through October 2025. This could not be corrected. Corrective Action: Effective 1/14/2026, S13 stated she will have S7 complete all of her trainings by February and will continue to offer incentives to make sure all staff complete their annual trainings in time and to ensure compliance with this regulation.

      Based on observations at 10:50 a.m., S4 was observed lifting C1, 1-year-old, by one arm. This was corrected while Specialist was present. Corrective Action: Effective 7/30/2025, S1 stated S2 had a one on one with S4 to reiterate proper lifting techniques and proper lifting will be discussed with all staff during their August staff meeting to ensure compliance with this regulation.

      Based on record review and interview at 1 :00 p.m., S1 failed to ensure documentation of the visual check of the vehicle was complete as it did not include the signature of the person conducting the check and the time the vehicle was checked on 3/18/2025-3/19/2025. This was not corrected. Corrective Action: Effective 7/30/2025, S1 stated she will go over the forms with S11 before the next transport, and she will check all forms weekly for accuracy once they are turned in to ensure compliance with this regulation.
