Based on record review at 12:30 p.m., S1 failed to have documentation of notification to the parent when the following occurred: On 02/24/2026 at 4:24 p.m., C1âs, age two-years-old, was guided out the door by S3, when he pulled away from her, running towards O1 at the front gate, C1 fell over the step at the front door scraping the left side of his face on the concrete. S3 wiped off C1âs face with a baby wipe. Corrective Action: Effective 3/9/2026 S1 stated she will check parental notifications and documentation of all incidents and accidents at the center at the end of each day…
