Based on record review/interviews at 1:16 p.m., although the center has a Behavior Management Policy implemented, S3 (DOH 9/8/2017) used a prohibited method of discipline as C1, age three-years-old, was subject to physical punishment: On 12/11/2024,while sitting on the floor S3 snatched a folder out of C1s hand, placed the folder in C1s school bag and shoved the school bag into C1s chest resulting in C1 falling backwards on his back.

1721.A.1 Based on record review/interview at 11:33 a.m. S1 failed to have documentation that the center staff of an early learning center, obtained a minimum of 12 clock hours of training annually in the topics found in 1719(A) and (B). 5 of 11 staff did not have the required continuing education training as of 11/12/2024. This is in addition to the required training from the Department of Health, Pediatric First Aid and Infant Child Adult CPR. The Specialist observed that S2, S3, S4, S7 and S11 did not have 12 clock hours of annual training.
