1907.E.2. Based on observations at 1:00 p.m., S1 failed to ensure the center's cribs were free of blankets, while a child was in the crib asleep. The Specialist observed a blanket covering C2, age one-years-old while she slept in her assigned crib. S5 removed the blanket during the licensing visit. Corrective Action: Effective 09/25/2025, S1 stated she will remind staff to remove blankets when infants fall asleep to ensure compliance with this regulation.

Based on record review and interviews on 4/8/2025, at 1:30 p.m., S9 and S12 failed to ensure children were supervised at all times as on 4/3/2025, at 9:20 a.m., C1, aged-3-years- old, walked out of the back door of the classroom. C1 was discovered in the parking lot of the center, by a person passing by on the street. S9 and S12 were responsible for supervising C1. C1 was unsupervised for 6 minutes.
