      Based on record review and interviews on 10/03/2025, S7 failed to supervise the children at all times and know the whereabouts of all children within her classroom. On 9/18/2025, at 3:50 p.m., C1 (one-year-old) exited the classroom by opening a door which led upstairs to S1's office. C1 closed the door after he entered the stairwell. She realized C1 was missing from the classroom after 2.5 minutes. S7 then retrieved C1 from S1s office upstairs at 3:52 p.m. to bring him back to the classroom. S7 was terminated on 9/18/2025.
