Children’s Files **The files that were checked by compliance staff during the compliance visit were mostly up to date. The provider was missing a physical in one child file and a immunization record in another file. Suggestions/Recommendations: Discussion with the provider on the types of forms or print off material from medical offices that are accepted by compliance staff so that the provider knows what to ask parents for annually. These forms will not need to be turned into compliance staff. You are encouraged to obtain these items from families as soon as you can and keep them in the files.
