      Several infants and ones did not have their feeding plan updated or the child recently turned 15 months and never had his feeding plan updated. Violation confirmed corrected by letter received from provider on 5/1

    • All Walls and Ceilings Including Doors and Windows Were Not Kept Clean, Free of Visible Fungal Growth, and in Good Repair.
