      to the Department by 6/26/23. The director will have a staff meeting about personal rights training and submit the agenda signed by staff to the Department within 30 days. The director will have the staff review the Department videos on Personal Rights. The director will submit LIC 624 (unusual incident report) 8 9 10 11 12 13 14 This is evidenced by: Child #1 was not treated with dignity by a staff member. This is an immediate risk to the health and safety of children in care. 8 9 10 11 12 13 14
