The facility failed to report a suspected communicable disease outbreak for two or more children or adults within 72 hours to the local county health department and failed to follow directions given. Director stated that she was not aware of reporting any communicable disease to the county health department. Specialist give technical assistant: Outbreaks A. Operators are required to notify the local county health department immediately upon any suspected outbreak of communicable disease and must follow the health departments direction. B. A suspected outbreak occurs when two or more children or child care personnel have the onset of similar signs or symptoms within a 72-hour period or when a case of a serious or reportable communicable disease is diagnosed or suspected in a child or employee. Some examples include shigella, salmonella, chicken pox, measles, pertussis, and hand, foot and mouth disease. Contact your local health department for a determination of whether reporting is required. The personnel/ volunteer (ten hours or more per month) record did not include a CF-FSP 5337 Child Abuse and Neglect Reporting Requirements form signed annually. CCF Handbook, Section 7.4, C Record Keeping [SR] Counselor observed that the facility record did not include a CF-FSP Child Abuse and Neglect Reporting was not signed annually. Provider called owner and owner via emailed the documents. Counselor observed personnel staff J.G. with expired Child Abuse Reporting on 08/26/2022. Counselor observed other personnel staff J.O. expired on 01/01/2019. The facility complies with Florida Standards at inspection. Counselor given technical assistance was provided: B. CF Form 1649A, Childcare Attestation of Good Moral Character, which is incorporated by reference in 65C-22.001(7)(a), F.A.C., must be completed for all child care personnel at the time of initial screening or upon change in employers. CF Form 1649A may be obtained from the Departments website at www.myflfamilies.com/childcare. C. A copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. D. A copy of the DCF letter/email informing of search conducted of the Floridas child abuse and neglect registry must be on record for each personnel screened between July 1st and December 15th of 2016. E. A copy of each request made to out of state child abuse and neglect registries for individuals who lived outside the state of Florida in the preceding five years.

Counselor given technical assistance was provided: 7.4.1 Background Screening Documents Background screening documentation must be maintained for all child care personnel as defined by Section 402.302(3), F.S., which includes household members if the facility is located in or adjacent to the home of the operator. Background screening documentation must be on-site and available for the licensing authority to review. A. Each personnel record must have a completed CF-FSP Form 5131, Background Screening and Personnel File Requirements, which is incorporated by reference in paragraph 65C-22.001(7)(b), F.A.C. B.
