Based on record review/observation at 2:30 p.m., S10 (DOH; 3/18/2025) failed to have current certification in pediatric first aid and CPR within 90 calendar days from the date of hire and prior to assuming sole responsibility for any children. This was corrected prior to the Specialist exit when S2 accompanied S10 in the classroom. Corrective Action: Effective 4/07/2025, S1 stated she will not allow any staff to be solely responsible for any children without pediatric first aid and CPR to ensure compliance with this regulation.

Based on record review at 1:00 p.m., S1 failed to document an explanation when something was observed, noted on children upon arrival to the center from January 2025 until April 7, 2025. Results including an explanation from parent and/or child were not documented. Corrective Action: Effective 4/07/2025, S1 stated she will follow up with parents to receive an explanation for any marks or bruises on a child to ensure compliance with this regulation.

C1 last authorization was dated 6/27/2024. C2 last authorization was dated 7/18/2024. S1 failed to have a written emergency medication plan of action for C3, age 1 year old, on file at the center. Corrective Action: Effective 4/07/2025, S1 stated she will update emergency medication plan of action in the months of April and September to ensure compliance with this regulation.
