Based on record review at 10:30 a.m., a CCCBC-based determination of eligibility for child care purposes from the Department failed to be obtained for S10, prior to being present at the center or performing services. According to daily attendance records, S9 worked from 10/22/2025 until 10/23/2025 and S10 worked from 9/29/2025, until 10/10/2025, without a CCCBC-based determination of eligibility. Based on observation at 10:25 a.m., The Specialist observed a plastic bag, not in use accessible to children in the two year old classroom. No one removed the bag prior to the Specialist exiting. Corrective Action: Effective 10/30/2025, S3 stated she will remind staff to remove plastic bags off the floor and away from children to ensure compliance with this regulation.

Based on observation at 10:25 a.m., The Specialist observed strings and cords accessible to children under the age of four in the two year old classroom. Corrective Action: Effective 10/30/2025, S3 stated she will try to arrange the furniture to make the cords inaccessible to children to ensure compliance with this regulation.

Based on record review/observation at 11:18 a.m., S1 failed to immediately notify the parents of C1, age 3 years old, following C1 falling and bumping her head on the refrigerator. On 9/29/2025, S10 failed to immediately notify the parents of C2, age 2 years old, when he fell and hit his head. Corrective Action: Effective 10/30/2025, S3 stated she or S1 will immediately notify the parents if or when a child have a neck or above injury to ensure compliance with this regulation.
