      A staff member left the classroom to use the adult bathroom which was located down the hall leaving one staff member with eighteen children age 4-5 years old. Violation confirmed corrected by follow-up visit made by consultant

      The red plastic sink in dramatic play area in classroom #204 was broken and jagged on 2 corners. Violation confirmed corrected by letter received from provider on 11/4/2024

      In classroom #206, I observed one staff member leave the room to get lunches for children in the cafeteria leaving one staff member in the classroom with seven (7) four-year old children and eight (8) five-year old children. Vi
