Areas of the facility were found to be in need of cleaning. CCF Handbook, Section 3.1, A (Section 2.1 Health and Safety, Page 3) Specialist conducted a kitchen inspection, kitchen was not clean and many items were left out from breakfast preparation. Many dishes were on and around washing area. Floor was not clean. Kitchen was not organized. Specialist had the director and kitchen staff clean and wash all dishes. Kitchen was cleaned during inspection. Per the Childcare Facility handbook on page 20. General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Director will email updated record to specialist after it's conducted. In the childcare facility handbook on page 27-28. During the facilitys license year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. One fire drill using an alternate evacuation route, 2. One fire drill during napping/sleeping times. 3. One fire drill in the presence and at the request of the licensing authority. DH Form 680 and DH Form 681 may be obtained from the local county health department. The parent/guardian of a child who has not received the age-appropriate immunizations prior to enrollment and who does not have documented medical or religious exemptions from routine childhood immunizations must provide documentation of a scheduled appointment or arrangement to receive immunizations. Director stated that the rugs will be cleaned this week. The Child Care Facility Handbook states: 3.1 General Health and Safety Requirements A. All child care facilities must be clean, in good repair, free from health and safety hazards and from evidence of, or presence of, vermin infestation. Indoor play areas must be inspected daily for basic health and safety and documented on a daily inspection log. Outdoor play areas must be inspected daily for basic health and safety. Any problems must be corrected before the play area is used by children.

Bottles and/or sippy cups brought from home were not labeled with childs first and last name. CCF Handbook, Section 3.9.5, B Sippy cups were not labeled with the child's first and last name. Staff wrote the children's names on their cups during this monitoring. The Child Care Facility Handbook states: 3.9.5 Food Handling A. Bottles and sippy cups provided by the facility must be washed, rinsed, and sanitized between each use and do not have to be labeled. B. Bottles and sippy cups brought from home shall be individually labeled with the childs first and last name and shall be returned to the custodial parent or legal guardian daily.

Child Care personnel, who were not in compliance with training requirements when they left the industry, did not complete required training and any new mandated training before returning to the industry. CCF Handbook, Section 4.3, B One employee, KW, did not complete the required training and returned to the industry out of compliance. Director stated she will be in compliance within the month. The Child Care Facility Handbook states: 4.3 Break in Service B. In the event an individual leaves the child care industry not in compliance with the training requirements and returns to the industry either at the same or a different child care facility, he or she must comply with the training requirements described in this section, as well as any new training requirements that may have been added during the gap in employment in the child care industry prior to re- employment.

The facility failed to maintain a current Employee/Contractor Roster for all child care personnel in the Clearinghouse. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. K. The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse within 10 days of when the individual has received a child care eligible result and has been hired at the facility. Employer/owner/operator must add an end date for individuals on the Employee/Contractor Roster in the Clearinghouse within 10 days of the employment termination.

A ratio of [1] child care personnel for [6] children is required. A ratio of [1] child care personnel for [7] children was observed. s.402.305(4), F.S. Staff stated that there were 2 teachers in the room with 7 one-year-old children and 1 teacher left the room to go to another room and left her alone with 7 one-year-old children. Staff stated that the teacher came back approximately 5 minutes later. Teacher apologized and stated she would not do that again. The 2021 Florida Statutes 402.305(4) states: (4) STAFF-TO-CHILDREN RATIO. (a) Minimum standards for the care of children in a licensed child care facility as established by rule of the department must include: 1. For children from birth through 1 year of age, there must be one child care personnel for every four children. 2. For children 1 year of age or older, but under 2 years of age, there must be one child care personnel for every six children. 3. For children 2 years of age or older, but under 3 years of age, there must be one child care personnel for every 11 children. 4. For children 3 years of age or older, but under 4 years of age, there must be one child care personnel for every 15 children. 5. For children 4 years of age or older, but under 5 years of age, there must be one child care personnel for every 20 children. 6. For children 5 years of age or older, there must be one child care personnel for every 25 children. 7. When children 2 years of age and older are in care, the staff-to-children ratio shall be based on the age group with the largest number of children within the group.

Child care personnel did not comply with the facility's written disciplinary and expulsion policies. CCF Handbook, Section 2.8, B A teacher placed her whole hand on a one-year-old child's head and forcefully pushed him to the side. Administrator suspended the teacher and reprimanded the teacher. The Child Care Facility Handbook states: 2.8 Child Discipline A. The child care facility shall adopt a discipline policy consistent with Section 402.305(12), F.S., including standards that prohibit children from being subjected to discipline which is severe, humiliating, frightening, or associated with food, rest, or toileting. Spanking or any other form of physical punishment is prohibited. B. The childcare facility operators, employees, and volunteers must comply with written disciplinary and expulsion policies.
