      Child(ren) did not have a Student Health Examination/DH (Form 3040), or an equivalent health statement on file within 30 days of enrollment. FDCH/LFCCH Handbook, Section 8.2, A Licensing Specialist observed two children missing a Student Health Examination record (Form 3040) on file. Technical assistance provided from the FDCH Handbook: 8.2 Student Health Records A. The operator is responsible for obtaining for each child in care a current, complete and properly executed DH 3040, Student Health Examination form, incorporated by reference in 65C-22.001(7)(q), F.A.C. and may be obtained from the local county health department, or a signed statement by an authorized professional that indicates the results of the components of the form are included in the health examination from the custodial parent or legal guardian, within 30 days of enrollment. Corrective Action: Provider may submit updated records via email to bring this standard into compliance.
