Based on record review, the licensee did not comply with the section cited above in Staff #4 who has been employed at the facility since June 2022 was not associated to the facility which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 02/15/2023

Based on record review, the licensee did not comply with the section cited above in one out of five children's files did not have documentation of immunizations or proper documentation for immunization exemption (C#3) which poses/posed a potential health, safety or personal rights risk to persons in care. LPA is noting that facility did have email communication with San Diego Public Health and a copy of Individual Education Plan (IEP) in the file stating the IEP will suffice as proof of child being exempt from immunizations.
