Child/ren were placed in a crib, playpen, play yard or other sleeping and napping bedding with items that could pose a strangulation or suffocation risk. CCF Handbook, Section 3.6.4, A (Section 2.1 Health and Safety, Page 3) Physical Environment [SR] Specialist observed that children in crib were covered with a blanket and had bottles in crib. Specialist had teachers remove all blankets and bottles from childrens cribs. In the childcare facility handbook on page 26. Cribs or playpens/play yards used for infants must have tight fitted sheets and no excess bedding, which includes but is not limited to: bumper pads, hanging mobiles, quilts, comforters, receiving blankets, pillows, stuffed animals and cushions. Supervision of children in the [3 year old classroom] group was inadequate in that [one teacher in the classroom was not supervising the children in her care.]. CCF Handbook, Section 2.4 General Requirements [SR] Specialist observed that one of the teachers in the classroom was not supervising the children while the other teacher was conducting circle time. Teacher will need to take the DCF training online class titled Challenging Behaviors and Awareness (CBAP). Per the Childcare facility handbook on page 12. General Supervision Requirements A. Childcare personnel must position themselves in the outdoor play area so that all children can be observed and directly supervised. B. CCF Handbook, Section 3.9.5, B Food and Nutrition [SR] Specialist observed that the Three-year-old room and the four-year-old/ VPK room did not have water bottles labeled with childs first and last name on them. Documentation of Level 2 Clearinghouse screening clearance was missing for child care personnel. CCF Handbook, Section 7.4.1,C Record Keeping [SR] Specialist reviewed that the cook did not have a cleared screening. Director sent her home until screening is cleared. Per Childcare facility handbook on page 62. 7.4.1 Background Screening Documents a copy of the eligible results, for the Level 2 screening, generated from the Clearinghouse must be on record for each personnel. If awaiting Out-of-State criminal history results, a copy of the DCF email informing of the individuals eligibility for a provisional hire status must be in the personnel file. Child Care personnel, who were not in compliance with training requirements when they left the industry, did not complete required training and any new mandated training before returning to the industry. CCF Handbook, Section 4.3, B One childcare employee who was not in compliance with training requirements when they left the industry in March of 2023, did not complete required training before returning to the industry. Director stated she has finished all of the courses, and she will complete the testing within the next 30 days. The Child Care Facility Handbook states: 4.3 Break in Service A. In the event an individual leaves the childcare industry in compliance with training requirements and returns to the industry either at the same or a different childcare facility, he or she will be given 90 days to comply with any new training requirements established during the gap in employment in the childcare industry. B. In the event an individual leaves the childcare industry not in compliance with the training requirements and returns to the industry either at the same or a different childcare facility, he or she must comply with the training requirements described in this section, as well as any new training requirements that may have been added during the gap in employment in the childcare industry prior to re-employment.

A fire hazard, [lint build up in heating and air vents, filters], was observed in the facility. CCF Handbook, Section 3.1, B (Section 10, number 2) Physical Environment [SR] A room had an air vent that had lint build up on the vent. Staff removed the lint from the air vent. Director stated they would change the air filters more often to stop the accumulation of lint and dust. The Child Care Facility Handbook states: 3.1 General Health and Safety Requirements B. It is the responsibility of the director/operator to ensure all areas and equipment of the facility are free from fire hazards, such as lint build up in heating and air vents, filters, exhaust fans, ceiling fans, and dryer vents. This includes grease build-up in ovens, stoves and food equipment.

The facility failed to have a test kit or other device to accurately measure the concentration of the sanitizing solution used for ware washing. CCF Handbook, Section 3.9.4, A.4. & C.3 Chemical sanitization could not be measured accurately as the incorrect testing strips were being used. Counselor left a tube of strips for this facility to use. C. Chemical Sanitization. 1. If chemical sanitization is used, an exposure time of at least 7 seconds is required for a chlorine solution of 50 mg/L that has a pH of 10 or less and a temperature of at least 75 degrees Fahrenheit. 2. If other sanitizers are used, the manufacturer instructions must be strictly followed. 3. A test kit or other device that accurately measures the concentration of the sanitizing solution must be available and used to confirm appropriate concentration of solution during each use.

The ground cover or other protective surface under the [swings and slides] was not maintained. CCF Handbook, Section 3.12, D (Section 11.2, number 3) Sanitation and Equipment [SR] The ground cover, mulch, was not maintained under the swings and slides in all playgrounds. Director stated they would order the mulch today. The Child Care Facility Handbook: 3.12 Outdoor Equipment D. Permanent or stationary playground equipment must have a fall/use zone that extends a minimum of 6 feet in all directions from the perimeter of the equipment. All types of ground cover must be maintained to provide resilience and reduce the incidence of injuries to children in the event of falls. 1. If the ground cover in place is loose ground cover (such as, but not limited to: mulch, shredded rubber chips, or sand) a minimum of 6 inches in depth is required in the use zone. Asphalt, concrete, hard packed dirt, hay, grass or leaves are unsuitable for use in the use zone area.

Child(ren) did not have a Florida Certification of Immunization (DH Form 680) or a Religious Exemption from Immunization (DH Form 681), on file within 30 days of enrollment. CCF Handbook, Section 7.1, B (Section 19.2, number 2) Record Keeping [SR] One child had an expired DH Form 680. Director stated she would acquire the form within the week. The Child Care Facility Handbook states: 7.1 Immunization Records The child care facility is responsible for obtaining for each child in care a current, complete and properly executed Florida Certification of Immunization form Part A-1, B, or C, DH 680, which is incorporated by reference in 65C-22.001(7)(o), F.A.C., or the Religious Exemption from Immunization form, DH 681, which is incorporated by reference in 65C-22.001(7)(p), F.A.C., from the custodial parent or legal guardian. DH Form 680 and DH Form 681 may be obtained from the local county health department. Specific immunization requirements are included and detailed in the most current edition of the Immunization Guidelines-Florida Schools, Childcare Facilities and Family Day Care Homes as promulgated by the Florida Department of Health. A. Immunizations received out-of-state are acceptable; however, immunizations must be documented on the Florida Certification of Immunization form and must be signed by a physician practicing in the State of Florida. The facility failed to maintain a current Employee/Contractor Roster for all child care personnel in the Clearinghouse. CCF Handbook, Section 5.1, K The facility's current roster was inaccurate in that they needed to remove two individuals and add two individuals. Director corrected the roster during this monitoring. The Child Care Facility Handbook states: 5.1 Initial Screening A screening must be conducted as a condition of employment. The employer/owner/operator must review each employment application to assess the relevancy of any issue uncovered by the complete background screening, including any arrest, pending criminal charge, or conviction, and must use this information in employment decisions in accordance with state laws. K. The employer/owner/operator must add child care personnel to their Employee/Contractor Roster in the Clearinghouse within 10 days of when the individual has received a child care eligible result and has been hired at the facility. Employer/owner/operator must add an end date for individuals on the Employee/Contractor Roster in the Clearinghouse within 10 days of the employment termination.

During the facilitys licensure year, fire drills utilizing the approved alarm system were not conducted monthly at various dates and times when children were in care [A fire drill had not been conducted since February.]. CCF Handbook, Section 3.8.4, A (Section 14.3, number 2), (Section 14.3, number 1) Physical Environment [SR] Upon review of the fire drill log it was discovered that a fire drill had not been conducted since February. A drill was conducted today with this counselor during nap time. There were 66 children and 13 staff members participating. The Child Care Facility Handbook states on page 27: 3.8.4 Fire Drills A. During the facilitys license year, fire drills utilizing the alarm system, approved by the local fire authority, must be conducted monthly at various dates and times when children are in care. C. A current attendance record and parent contact information must accompany child care personnel out of the building during a drill or actual evacuation and be used to account for all children. The operator must maintain a written record of the fire drills showing the date, number of children and child care personnel in attendance, evacuation route used, and time taken for all individuals to evacuate the premises. Each fire drill record must be maintained for a minimum of 12 months from the date of the fire drill. The fire drills conducted must include, at a minimum: 1. One fire drill using an alternate evacuation route, 2. One fire drill during napping/sleeping times, and 3. One fire drill in the presence and at the request of the licensing authority.
