      Based on interviews at 11:00 a.m., S11 and S12 failed to ensure children were supervised at all times. On 4/7/2026, at around 5:09 p.m., C1, 3-years-old, was left unsupervised on the playground for approximately one minute. This was corrected when S11 went back to the playground and retrieved her. Corrective Action: Effective 4/14/2026, S1 stated she has informed staff to do their head count outside while children are lined up and before going inside to ensure compliance with this regulation.

      Based on record review at 10:00 a.m., S1 lacked documentation of a daily attendance record for children that included the time of arrival and departure of each child and the first and last name of the person to whom the child was released. C1, 3-years-old and C2, 4-years-old, were present on 4/7/2026, but were not signed in or out. This could not be corrected. Corrective Action: Effective 414/2026, S1 stated she will send a message to all parents reminding them to sign children in/out at the kiosk using their 4-digit code and she will check attendance between 9-9:30 a.m. daily to…
