      Based on record review, the licensee did not comply with the section cited above. In reviewing staff records Staff#3 did not have proof of immunization on file. This poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 05/08/2024

In reviewing staff files, LPA observed that Staff #1 and Staff #4 had not completed the Mandated Reporter Training as required by AB1207.
