      Based on record review at 8:30 a.m., S1 failed to have all current staff names, hire date, first day onsite working with children, and initial and annual mandatory reporter training documentation maintained in the LDOE electronic system. Corrective Action: Effective 6/10/2026, S2 will notify S1 to immediately add all staff names, hire date, first day onsite working with children, and initial and annual mandatory reporter training documentation in the LDOE electronic system to ensure compliance with this regulation.

Based on record review at 1:13 p.m., S1 failed to have documentation that 1 of 10 staff, S5, completed the online child abuse and neglect Mandated Reporter Training provided by DCFS annually. The most recent Mandated Reporter Training for S5 was completed on 10/16/2024. S5 failed to complete the training in 2025. Corrective Action: Effective 3/11/2026, S1 will require all staff to complete the Mandated Reporter Training in August each year to ensure compliance with this regulation.
