LPA recommends to the Licensee, to complete a written statement, what have transpired in this violation and what is plan moving forward. The Licensee will submit a POC proof to the Regional Office on or by the POC due date. LPA encourage the licensee to reach out to the LPA as needed. 8 9 10 11 12 13 14 This requirement is not met as evidenced by: On LPA Estoesta visit conducted on 05/20/2025, the Licensee Krishna Kant, provided disinformation to LPA Estoesta. During LPA’s interview on the 05/20/2025 visit, S1 and S2, were the individuals who were misrepresented to LPA Estoesta. S1 and S2 claimed the identity of individuals who previously worked in the facility. On 05/22/2025, the Licensee spoke with LPA Estoesta over the phone. The Licensee Krishna Kant, confessed that S1 and S2 were not the individuals who claimed they were to be. Based on the above evidence, the licensee did not comply with the section cited above which poses an immediate health, safety and personal rights risk to children in care. 8 9 10 11 12 13 14 The Regional Office will send an invitation to the Licensee of an in person - Informal Meeting to the Regional Office.
