Based on record review at 12:30 p.m., S1 failed to have a center's daily attendance record for children that included the child’s time of departure and first and last name of person or entity to whom the child is released. On 3/5/2026, 8 of 14 children did not have documentation that accurately reflect the time of departure and the first and last name of person or entity to whom the child is released. Corrective Action: Effective 3/12/2026, S2 stated she will remind parents and staff to check the attendance for documentation to ensure compliance with this regulation.

Based on record review/interview at 12:45 p.m., S1 failed to have documentation of immediate notification to the parent when the following occurred: On 3/4/2026, time unknown, C1, age 5-years-old, stuck a sequin up his nose while on the outdoor play yard. The parent was notified at pick up. Corrective Action: Effective 3/12/2026, S2 stated she or S3 will complete all head or neck or eye injuries notifications to parents immediately to ensure compliance with this regulation.
