      I observed three (3) ceiling tiles along the wall where the couch is. Violation confirmed corrected by letter received from provider on 6/17/2025

    • The Indoor And/or Outdoor Premises Was Not Checked Once a Day, Prior to Initial Use, Ensuring Debris, and Broken Equipment Was Removed and Disposed Of.

      In space #1, I observed a piece of art with a broken frame on top of a shelf. Under that, on the shelf, I observed a broken toy bin.
