      2101.A.1. Based on record review and interview at 12:15 p.m., S1 failed to ensure transportation arrangements conformed to state laws, including seat belts and child restraints. On 11/14/2025, S1 and S5 transported 10 children, ages 3 to 4-years-old, according to Field Trip Transportation log, in a Chevrolet suburban without a forward-facing car seat with a harness for each child that is required by the Louisiana state law for children ages 2 to 4-years-old. Corrective Action: Effective 11/20/2025 S1 stated she will have the parents leave the children’s car seats at the center for field…

      2101.A.2. Based on record review and interview at 10:30 a.m., S1 failed to restrain only one child in a single safety belt. According to the Field Trip Transportation log, on 11/14/2025, 10 children, ages 3 to 4-years-old, were transported in an 8 passenger Chevrolet suburban. S1 stated she did have 4 children share a safety belt. Corrective Action: Effective 11/20/2025 S1 stated she will contract a transportation vehicle that provideshaveenough safety belts for each child to ensure compliance with this regulation.
