      Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. Licensing Program Analysts (LPAs) observed that the lining on the playground structure's stairs was broken and jagged in some areas, leaving sharp and pointed parts exposed. POC Due Date: 03/06/2026

      Based on record review, the licensee did not comply with the section cited above in that LPA observed that Staff 2, Staff 4, and Staff 5 did not have a personnel file on site. LPA observed that Staff 2 had taken the incorrect Mandated Reporter, and that Staff 4 and Staff 5 did not have a Mandated Reporter on file. LPA was informed that Staff do have a file, yet are located off site, which poses/posed a potential health, safety or personal rights risk to persons in care. POC Due Date: 11/20/2023

      Based on record review, the licensee did not comply with the section cited above. During today's inspection, all children's files were missing signed LIC 613A. Lead teacher stated it was not given to parents which poses a potential health, safety or personal rights risk to persons in care. POC Due Date: 01/21/2022
