Based on observation, the licensee did not comply with the section cited above which poses a potential health, safety or personal rights risk to persons in care. During the indoor inspection, Licensing Program Analyst (LPA) observed that the main blue carpet in Clasrooms 1 and 2 had dark marks and staining throughout. LPA observed some dead insects/flies along the bottom of the windows/window sills in Room 2. LPA also observed some ants on the cubbies near the front entrance in Room 1 and around the outside of the door in Room 2. Lastly, LPA observed a strong odor of urine coming from the bathroom in Room 1. POC Due Date: 12/15/2025

Based on observed, the licensee did not comply with the section cited above in LPA observed that an uncleared adult was in the Staff office area in Classroom 1. LPA was informed by Staff #3 that adult is not fingerprint cleared or a volunteer.
