Director also agreed to create an agreement between the Parents of (C6) and the facility and submit the plan to LPA by POC due date. 8 9 10 11 12 13 14 Based on interviews from staff, parents, records review, and picture observation it has been determined Child (C6) has caused injuries to multiple children in care.
