A.8.a.b.: Based on interviews on 05/11/2026, at 9:49 a.m., although the center has a written behavior management policy implemented, S3 (DOH 12/20/2022), used a prohibited method of discipline as C3, age 1-year-old, was subject to physical/corporal punishment. On 05/04/2026, between 11:00 a.m. and 12:00 p.m., S3 forcefully placed C3, age 1-year-old, in the bouncer. After placing C3 in the bouncer, she forcefully shook the bouncer with her foot. 1713.E.&F.: Based on interviews on 05/11/2026, at 9:49 a.m., while supervising a group of children, in the classroom on 05/04/2026, at 5:03 p.m., S3 failed to devote her time to supervising children and participating with the children in their activities; S3 was on the centerâs computer while C1, age 10-months-old, was choking on a blue piece of tape from his bottle. Corrective Action: Effective 05/21/2026, S1 stated she will conduct a meeting every other month in regards to active supervision to ensure compliance with the regulation.

Corrective Action: Effective 05/21/2026, S1 stated she will get written authorization prior to using boppy pillows and positioning devices for infants to ensure compliance with the regulation.

1719.B.Based on record review at 10:30 a.m., S1 failed to have documentation that S6 (DOH 3/13/2025), S9 (DOH 03/13/2025) and S10 (DOH 1/9/2025) completed the DCFS online Mandated Reporter Training within seven days of the first day present at the center. Corrective Action: Effective 5/7/2025, S1 stated she will have all new hired staff complete required training with first 5 days present at the center to ensure compliance with this regulation.
